PRE-K, KINDERGARTEN STUDENTS LEBANON -- Registration for children applying for the Pre-Kindergarten program and for children attending Kindergarten within the Lebanon Special School District (Byars Dowdy Elementary, Coles Ferry Elementary, Sam Houston Elementary, Cumberland University Pre-K and Market Street Pre-K) will be on Friday, May 8 for the 2009-10 school year. Registration will take place at College Hills Church of Christ, 1401 Leeville Pike, Lebanon, from 11 a.m. until 6 p.m. The Pre-Kindergarten program is voluntary and eligibility is based on income level and age. Children must be 4 years of age on Sept. 30 and zoned for LSSD in order to apply. Parents of Pre-Kindergarten applicants should contact Penny Thompson (Central Office, 701 Coles Ferry Pike, 449-6060 ext. 24) if they are unable to attend registration on May 8. Kindergarten students must be 5 years of age on or before Sept. 30 to be eligible to attend Kindergarten. Parents of Kindergarten students should contact the school they are zoned to attend if they are unable to attend registration May 8. They are Byars Dowdy Elementary 444-6651; Coles Ferry 8ementary 443-1946; and Sam Houston Elementary 444-7494. All students must have the following prior to school entrance in July:- Original Birth Certificate - Immunization Record (from Health Dept. or Doctor) - Social Security Card - Proof of Residence - *Proof of Insurance (Pre-Kindergarten applicants only) The school physical can be obtained during registration if desired. Vision Screenings (Lion’s Club), hearing evaluations and dental screenings will be provided at no cost. The Health Department will not be available to give free immunizations this year, however "walk-in" clinic service will be available from 8-11 a.m. and from 1-3 p.m. on May 8 only at the Health Department.
