State is slow to comply with violation report

The administrator at Lebanon Health and Rehabilitation told The Post Wednesday that the parent organization would answer a series of questions regarding the violations, but as of press time Thursday, they too had failed to respond.


The investigation took place from July 29 through Aug. 5 with several immediate jeopardy violations being cited by the TDOH, according to Andrea Turner, a spokesperson for the department.


The findings in the departments report regarding the investigation were said to include the nature of the violations.


Im told theyre still working to finalize the survey report, said Shelley Walker, communication and marketing coordinator, in an email on Wednesday.


Walker indicated on Wednesday that TDOH staff sent a report that was a response from the facility (Lebanon Health) to the survey report, adding it doesnt really make sense without the accompanying report.


That second report includes details of the facilitys violations compiled by TDOH officials during the weeklong investigation. On Thursday, Turner said the TDOH staff was still finalizing the report.
